The PHQ-9 is a nine-item scale for screening and measuring depression severity. It asks how often you have been bothered by nine common depression symptoms over the past two weeks, rated from not at all to nearly every day. Scores range from 0 to 27.

0-4Minimal
5-9Mild
10-14Moderate
15-19Moderately severe
20-27Severe
